The Maryland Information Network (MdInfoNet) is a public-benefit nonprofit that strengthens information access, aligns social systems, and generates insights into service needs and gaps. 

We work with state agencies, hospitals, providers, legislators, and community and nonprofit organizations. MdInfoNet is also the administrator of the 211 system.

Case Study: Hospital Transitions Program

This 1-year pilot addressed hospital readmissions. They're preventable, yet they're one of the most pressing challenges facing Maryland's health and human services among Medicaid- and Medicare-eligible older adults and adults with disabilities. The program delivered intensive, person-centered support for 120 days after hospital discharge.

Care coordinators helped with waiver enrollment, insurance navigation, and access to essential services. Within the first month, hospital visits dropped 23%.

The pilot, sponsored by the Maryland Department of Aging and the University of Maryland School of Medicine, connected patients, improved dignity, and independence. It proved that targeted community stabilization can save money and change lives.

ConnectCare for Hospitals and Patients

The Care Coordination program is also reducing hospital readmissions among behavioral health patients, both adults and children.

Children ages 13-17 make up the majority of referrals. These complex cases reduce hospital boarding through a carefully coordinated referral process. The Care Coordinators connect patients to treatment and provide additional community resources.
